Mafia: The Old Country received a new update on September 1, and it targets the problems that can stop a playthrough rather than adding another story chapter. The patch fixes Free Ride freezes, missing progression rewards, companion behavior, and several smaller issues in the Journal and mission interface.
2K’s official notes do not give the update a version number. MP1st identifies the build as 1.011, but the safest name for the patch is still the September 2026 update used by 2K and Steam.
Free Ride is the optional mode for exploring Valle Dorata outside Enzo’s main campaign. It includes races and combat challenges, so a problem with a checkpoint or a transition can affect a short activity without ruining the story save. That explains why the notes spend so much space on Free Ride even though the patch is not adding a new mode.
Free Ride and Story Mode should stop trapping players
The most obvious fix is the transition from Free Ride back to Story Mode. 2K says it addressed cases where players became stuck during that switch. The update also improves mission progression and checkpoint behavior across Free Ride activities, so a failed attempt should be less likely to leave the mode in a broken state.
Free Ride races get separate stability work. The notes mention a better overall experience during races, while race times should now appear consistently during time trials. That is a small interface change, but it matters when the mode is built around beating a previous time.
Progression rewards and companion behavior were also fixed
Some players were finishing story content or meeting a progression condition without receiving the reward they were owed. 2K says the update addresses several versions of that problem. It does not list every affected reward, so the notes do not support a promise that a particular missing item will be restored automatically.
The patch also covers companions who failed to perform a required action and blocked mission progress. That change applies across multiple story and Free Ride situations. When an ally stops moving or misses a scripted task, reloading a checkpoint can be the only way forward; these fixes are meant to prevent those situations from happening as often.

New-game carryover is finally clearer
Previously unlocked outfits, hats, and haircuts are now available when starting a new game. This is useful for players replaying the campaign or beginning again after spending time in Free Ride. It is a carryover change, not a new customization system, and the patch notes do not mention weapons or other collectibles receiving the same treatment.
The Journal and mission icons received attention
2K improved Journal functionality, including chapter descriptions and the visibility of entries across different playthrough states. The update also changes how interaction icons look and behave during story missions. Those fixes will not alter the story, but they should make it easier to tell what Enzo can interact with and which parts of the campaign have already been recorded.

Visual glitches and AI issues are part of the cleanup
The update addresses unwanted geometry appearing during gameplay and includes additional improvements to AI behavior, mission progression, and overall stability. 2K describes these as fixes and polish rather than a new graphics mode. There is no 60 fps addition, new mission, or new Free Ride activity listed in the September notes.
This is a repair patch, not new content
The September notes do not announce a new chapter for Enzo, another character, or a new challenge set. The work is aimed at the existing campaign and Free Ride mode. That makes the update less exciting than a content drop, but it also makes the purpose clear: players should lose fewer runs to a broken transition, missing reward, or stalled companion.
Should you update before returning to Mafia: The Old Country?
Yes, especially if Free Ride has left you stuck, a companion has stopped a mission, or a reward failed to appear. The update is more useful for removing friction than for changing how the game plays. Players looking for new story content should not expect that from this patch; the focus is getting existing systems to finish what they started.
2K has not listed the download size or a platform-by-platform change list in the official notes. The notes are written as a shared update for the game, while MP1st reports the build as 1.011 on PlayStation 5, Xbox Series X|S, and PC. If your platform shows a different build label, the fixes above are still the changes described by the publisher.
