Mafia: The Old Country follows Enzo Favara, who survives indentured labor in Sicily’s sulfur mines and sees a chance to join Don Torrisi’s crime family. Set in the early 1900s, it is an origin-era story in the Mafia universe rather than a remake of the first game. The narrative centers on Enzo’s oath, the cost of belonging to the family, and the rivalries surrounding the Torrisi organization.
Hangar 13 built it as a focused, linear third-person action-adventure rather than a sprawling open-world checklist. Missions combine high-stakes stealth, knife fights, gunplay, horseback travel, and period vehicles. That structure helps separate the core release from later additions: the August 8, 2025 base game tells Enzo’s main story on PC, PlayStation 5, and Xbox Series X|S, while Man of Honor is a later expansion.
