Its single-player campaign moves between the Cold War era and a near-future conflict set in 2025. Alex Mason's Cold War story connects to David Mason's fight against Raul Menendez, a leader whose plan turns political conflict into a global crisis. Player decisions can change missions, character outcomes, and the ending. The campaign also includes Strike Force missions, which let players direct units and choose how to approach objectives instead of following one fixed route.

Black Ops II also has competitive multiplayer built around custom classes, the Pick 10 loadout system, scorestreaks, team objectives, and online progression. Weapons, attachments, perks, and equipment can be combined within the class limits, giving each player a distinct role. The Zombies mode adds cooperative survival, dedicated maps, its own progression, and several ways to play, including the larger Tranzit experience and competitive Grief variants.

The campaign, multiplayer, and Zombies are separate parts of the same 2012 package. The later PS4/PS5 release expands where the original game can be played but does not replace its original design with a new campaign or modern remake.
